Lahey is working with other hospitals and organizations to conduct surveys, interviews, and focus groups, and review existing information, to better understand the current health status of the community and prioritize strategies based on what matters most to people. They will finalize the report in  and will publish a report with the findings, which will be used to implement strategies prioritized by the community.

 

The goal is to make healthier communities for all! This survey is anonymous and takes fifteen minutes. By completing the survey, you will be entered in a drawing for a chance at winning a $100 Visa gift card. 

 

The survey is translated into 11 languages including Armenian, Cape Verdean Creole, Simplified Chinese, Traditional Chinese, Haitian Creole, Hindi, Khmer, Portuguese, Russian, Spanish and Vietnamese. The survey is located here and we encourage all to take a moment to fill it out and share your voice

FY25 Community Health Needs Assessment

Beth Israel Lahey Health and its member hospitals are conducting a Community Health Needs Assessment to better understand the most important health-related issues for community residents. Each hospital must gather input from people living, working, and learning in the community. The information collected will help each hospital improve its patient and community services. 

Please take about 15 minutes to complete this survey. Your responses will be private.  If you do not feel comfortable answering a question, you may skip it. Taking this survey will not affect any services that you receive. Findings from this survey that are shared back with the community will be combined across all respondents. It will not be possible to identify you or your responses. Thank you for completing this survey.
